Connect for Health Colorado
families can seek insurance coverage and also apply for Medicaid and Children’s Health Plan (CHP+) coverage.

Child Health Plan Plus (CHP+) – A low-cost health insurance program for uninsured Colorado children
ages 18 and under whose families earn or own too much to qualify for Medicaid but cannot afford private
insurance.

Citizens for Patient Safety (CPS) works for better healthcare outcomes by increasing engagement between patients/families and healthcare providers. Offers training in shared decision-making and informed consent. A program of Ability Connection Colorado.

Colorado Cares Rx is a program for Colorado residents to save money on generic prescription
medicines.

Early and Periodic Screening, Diagnosis, and Treatment (EPSDT) – a health care benefit package for all Medicaid enrolled children ages 20 and under and all pregnant women.  EPSDT now includes behavioral health services, including Applied Behavioral Analysis (ABA) therapy for children with a documented need.

Family Voices – Provides help with health insurance plans, communicating with service providers, navigating Medicaid or other public systems, individual health insurance counseling (such as in benefits decision or help with the appeals process). 1-800-881-8272

Find a Provider Agency – Information on agencies providing care services in the home or community for Colorado Medicaid clients.

The Health Care Program for Children with Special Needs (HCP) is a resource for families, health care providers, and communities. HCP seeks to ensure access to integrated, family-centered, culturally competent, community-based programs and services for families with children and youth with special needs. HCP Care Coordination provides additional supports and specialty clinics for families in mountain and rural areas.

Healthy Communities – Provides a Family Health Coordinator in your area who can help you access public health programs, find a medical home and coordinate care across programs.  Check this list for your local coordinator.
